The Prognostic Value of Cardiac Troponin in a Contemporary Cohort of Patients With First-Time Myocardial Infarction

The prognostic value of cardiac troponin following myocardial infarction (MI) is well documented; however, the benefit in a predominantly revascularised cohort less certain. This study reported role peak Multi-Ethnic New Zealand Study Acute Coronary Syndromes (MENZACS). Due to varying assays across hospitals, data from index admission were standardised as divided by upper limit normal (ULN) for each assay (“peak ratio”). Troponin ratio was categorically investigated (≤1, >1–10, >10–100, >100 over ULN) and continuous variable. relationship between all-cause death or cardiovascular readmission using Cox regression including restricted cubic splines. Models adjusted MENZACS clinical summary score (comprising 20 factors age sex), ethnicity, log2 N-terminal pro B-type natriuretic peptide (NT-proBNP). A total 1,876 patients admitted with first-time MI 2015 2019 included (mean 61 years, 21% women, 43% ST-elevation MI, 57% non-ST elevation MI); 99% received coronary angiography 88% during admission. Peak 1% ≤1, 19% 36% >10–100,and 44% >100. NT-proBNP increased increasing category. Overall, 20% died readmitted median 3.5 years. After multivariable adjustment, not associated (Figure 1). an independent predictor highly surviving MI.
